The milestone that matters most is straightening, not bending
People arrive focused on how far the knee bends, because that is the number the physiotherapist measures and writes down. Bending matters. But a knee that does not fully straighten is the one that gives trouble for years — you cannot walk normally on it, and the quadriceps never gets a chance to work properly.
So in the first weeks, full extension comes first. It is uncomfortable, it is not optional, and it is the thing most worth being disciplined about.
Your thigh muscle will not switch on, and that is normal
Almost everyone experiences this and almost everyone finds it alarming: you tell the quadriceps to contract and very little happens. That is arthrogenic muscle inhibition — the nervous system reduces the signal to the muscle after knee surgery. It is near-universal, it is not weakness in the ordinary sense, and effort alone does not fix it.
It is exactly what Neubie therapy targets. The current stimulates the muscle directly, re-establishing the signal so that the strength work you do afterwards actually lands. Without that step, people spend months doing exercises for a muscle that is not receiving the instruction.
Swelling is the pacing signal
A knee that is more swollen the morning after a session was loaded too hard. A knee that is the same or better tolerated it. That is a more reliable guide than pain, which is confounded by medication in the early weeks.
