Gluteal Tendinopathy Treatment in West End, Brisbane

Pain on the outside of the hip that makes lying on that side impossible — and that is usually mislabelled as bursitis.

Gluteal tendinopathy is the most common cause of pain over the bony point at the side of the hip. It is frequently called bursitis, which points the treatment in the wrong direction: the bursa is rarely the primary problem, the tendon is.

What you are likely to notice

  • Pain over the outside of the hip, on the bony point
  • Cannot lie on that side at night — often the worst symptom
  • Pain on standing on one leg, stairs and walking
  • Worse after sitting with legs crossed

What usually causes it

  • Compression of the tendon from positions that bring the knee across the midline — sitting cross-legged, standing with the hip dropped, sleeping with the top leg forward
  • A sudden increase in walking or hill work
  • Reduced gluteal strength, particularly around menopause

How we treat it at Forge

First, remove the compression — and the sleeping and sitting advice here changes symptoms faster than anything else we do. Then progressive gluteal loading. Shockwave therapy is well supported for this tendon when it has been stubborn.

How long it takes

Night pain often improves within weeks once compression is addressed. Full tendon capacity takes months.

Questions

What people ask about this

Is it bursitis?

Usually not, or not primarily. Imaging often shows bursal changes, but the tendon is the driver — which is why cortisone into the bursa gives short-lived relief and loading gives lasting change.

How should I sleep?

On your back, or on the unaffected side with a pillow between your knees so the top leg does not drop across. This one change frequently makes the biggest early difference.

Why did this start around menopause?

Tendon tissue is sensitive to hormonal change, and gluteal tendinopathy is notably more common in women in their fifties. It is not inevitable and it responds to loading.
