An anterior cruciate ligament injury is one of the more serious knee injuries in sport. Some people do well without surgical reconstruction; others need it. What is not optional in either case is a structured, measured rehabilitation program, because return to sport too early is the single biggest predictor of doing it again.
Restoring full knee extension and quadriceps activation first, because nothing works until those return. Neubie therapy has a specific role here: after an ACL injury or reconstruction the nervous system frequently keeps the quadriceps switched down for months, and no amount of loading works until the signal is back. Then progressive strength, then plyometrics, then sport-specific return, with objective testing at each stage.

No. A proportion of people manage well without reconstruction, particularly if they do not return to pivoting sport. That decision is made with a surgeon, and rehabilitation is required either way.
