Heel pain that is worst on the first steps of the morning — and one of the clearest cases for shockwave therapy.
Plantar fasciitis (more accurately plantar heel pain) involves irritation of the thick band of tissue under the foot where it attaches to the heel. It is common, it is stubborn, and it is one of the conditions where the right combination of loading and technology genuinely shortens the course.
Loading is the core of it: calf and foot strengthening, progressed carefully. Almost never. Heel spurs are common in people with no heel pain at all, and removing one is not the treatment. The pain comes from the tissue, not the bone.
Usually not stop — reduce, then rebuild. Complete rest lets the tissue lose the capacity it needs, and the pain comes straight back when you resume.
Sometimes, and a gait scan is how we find out rather than guess. Orthotics work best alongside a loading program, not instead of one.
