With a GP Chronic Condition Management Plan you can claim a Medicare rebate on physiotherapy and exercise physiology at Forge West End.
A GP Chronic Condition Management Plan (GPCCMP, previously called a GP Management Plan) lets eligible patients claim Medicare rebates on allied health care — including physiotherapy and exercise physiology — for conditions that have been present for six months or longer.
Your GP writes it. They set health goals, review your treatment and coordinate care with the health professionals involved. In some cases they will also create a Team Care Arrangement. The number of Medicare-rebated allied health sessions is limited per calendar year, and your GP confirms what applies to you.
Medicare physiotherapy is item 10960. Medicare exercise physiology is item 10953. Your referral needs to name which one you are being referred for — if you are not sure which suits you, the comparison below should help, or just call us.
Ask whether you are eligible for a GPCCMP. Your condition generally needs to have been present for six months or longer.
You need a copy of the plan and a referral for physiotherapy or exercise physiology. Email both to info@forgewestend.com.au or bring them to your first appointment at Unit 4, 249 Montague Road.

We do not bulk bill. You pay the fee in full on the day by EFTPOS, and Medicare returns the rebate directly to your bank account, electronically or through HICAPS. Fees are on the pricing page.
