Medicare (GPCCMP) West End services at Forge West End allow eligible patients to access Medicare rebates for allied health care, including physiotherapy and exercise physiology. If you have a GP Chronic Condition Management Plan (GPCCMP), also known as a GP Management Plan (GPMP), you may be able to receive coordinated treatment for long-term conditions under Medicare.
A GP Chronic Condition Management Plan is a Medicare-funded plan created by your GP for conditions that have been present for six months or longer. Your GP develops a personalised plan, sets health goals, reviews treatment, and coordinates care with relevant health professionals. In some cases, your GP may also create a Team Care Arrangement (TCA).
With the right documentation in place, Medicare may provide rebates for allied health services such as physiotherapy and exercise physiology. Medicare-rebated allied health sessions are typically limited each calendar year, and your GP will confirm what applies to you.
Choose your service:
Ask your GP whether you’re eligible for a GPCCMP/GPMP (your condition generally needs to be present for six months or longer).
Ask your GP for a copy of your GP Chronic Condition Management Plan (GPCCMP / GPMP) and a referral for physiotherapy or exercise physiology.

We do not offer bulk billing. Your appointment fee is paid in full on the day via EFTPOS. The Medicare rebate is then processed and returned directly to your bank account or, either electronically or via HICAPS.
