NDIS Services at Forge

At Forge, our clinicians appreciate the opportunity to make a lasting difference in someone’s life. As university qualified allied health professionals, they have the knowledge, skills and competencies to help people living with disability improve their overall health, through individualised safe and effective exercise programs.

Our integrated gym and clinical pilates studio offers Forge clinicians the ability to create exercise programs for a wide range of disabilities. It also allows for the easy modification of exercises if necessary due to a symptom flare-up or altered presentation on the day of the appointment. Our clinicians aim to build a trusting relationship with the client and will always act in their best interests.

NDIS services offered at Forge include:

  • In-clinic Manual Therapy and/or Program Design
  • Home Program Design
  • One on One Exercise Sessions
  • Small Group Therapy Sessions

In some circumstances our clinicians also have the ability to come to your house to deliver the exercise program if this would be more comfortable for the client (includes additional travel fee).

We understand that every client presents differently, and our Physiotherapists and Exercise Physiologists will work together to provide the most effective treatment plan for the client.


Whether you are currently suffering from an injury, acute or chronic or just want to exercise in a safe and supportive environment, our Individualised Physiotherapy led exercise programs are here for you. Your Physiotherapist will first conduct a thorough Functional Movement Assessment and then design a suitable program to address your specific goals using evidenced based exercise therapy.

Our Physiotherapy Exercise Classes can help you with both Pre and Post Rehabilitation of injuries, help you work on your strength and conditioning, mobility and flexibility as well as balance and proprioception.


Forge’s initial 60min NDIS appointments enable our Exercise Physiologists or Physiotherapists to conduct a thorough subjective assessment (including medical history and symptoms, previous and current exercise, and goals) as well as a physical assessment looking at functional strength, range of motion and movement patterns. This is to ensure that our clinicians have an in-depth understanding of your health to be able to prescribe an individualised and effective program. During this session, your clinician will also work with you to complete our NDIS Service Agreement document. This will outline your treatment plan whilst at Forge.


Upon the completion of your initial assessment, your Exercise Physiologist or Physiotherapist will begin to design your individualised exercise program. Each NDIS client requires a different level of care; if your clinician believes a group environment is suitable for your long-term management, they will guide you through this program during a small group training session. Quality of service is of upmost importance to Forge, and we limit these groups to a maximum of 4 people so our clinicians are able to ensure each client is working towards their own individual rehabilitation goals whilst working technically correct. Our goal is to build a trusting relationship with each of our clients and give them the skills and confidence to be more active and independent within their daily lives.


One on one Exercise Physiology sessions provide you with the opportunity for individualised guidance through your exercise session, which is designed around your initial physical assessment results and goals. During your sessions you can expect to be guided through correct movement technique to ensure you are exercising safely and effectively. Your clinician may also devise a home exercise program to further assist you in reaching your goals outside of the clinic. Our Exercise Physiologists also have the knowledge and expertise to adjust your program in the event that you have a flare up of symptoms or need to modify a certain movement during that session.


One on one Physiotherapy sessions can include manual therapy, exercise, or a combination of both. It provides you with an opportunity for individualised hands-on care and guidance through an exercise session which is developed around your initial physical assessment results and goals.

Together with your clinician you will devise a treatment plan that works best for you, which may include a home exercise program to further assist you in reaching your goals outside of the clinic. Our Physiotherapists also have the knowledge and expertise to adjust your program in the event that you have a flare up of symptoms or need to modify a certain movement during that session.



New clients are required to fill out an initial NDIS information form prior to your first appointment. This will be emailed to you as part of our ‘NDIS at Forge’ package once a booking is made. We recommend that you arrive at least 10 minutes before your appointment to review this form with the Receptionist, to ensure all necessary information is present and correct. Please be aware that there may be other clients in the studio working with our other therapists at the time of your assessment. Forge requires that you also bring a water bottle and towel to all subsequent gym-based appointments.


Please wear soft and comfortable clothing that allows you to move freely and easily. During your initial assessment, you will be in a private room with an Exercise Physiologist or Physiotherapist. During your appointment you will be required to complete a range of physical assessments, so it is important you dress appropriately.
